Neighborhood Health provides care, support and connection to resources based on each person’s needs. At your visit, our care team will talk with you about your health concerns, answer questions and recommend next steps.

Care that’s open to everyone

Neighborhood Health is open to people of all ages, from infants to older adults. Insurance is accepted, but not required in order to be seen.

We welcome people who face barriers to traditional care, including people who are uninsured, underinsured, unhoused, without reliable transportation or unsure where to go for medical help.

What to expect at your visit

1. Check in.

A public safety officer will greet you when you arrive, and a patient service coordinator will help you check in. If you have ID or insurance information, you can share it at this time.

2. Meet with the care team.

A nurse or medical assistant will ask about your health history, medications, allergies and reason for your visit. They may also check your vital signs, height and weight.

Your provider will talk with you about your health concerns, answer questions and recommend care based on your needs.

3. Receive care on site.

Depending on your needs, you may receive bloodwork, point-of-care testing, vaccines, wound care, minor procedures, medication or prescriptions during your visit.

4. Leave with a plan.

Before you leave, the team can help schedule follow-up appointments and connect you with community resources. You’ll also receive an after-visit summary with recommendations, next steps and any upcoming appointments.

Language And Translation Support

Bilingual team members are available at many Neighborhood Health clinics. Translation support is also available through Proprio. Haitian Creole support may be available on site at select locations.

How much does it cost?

You do not need insurance to be seen. Insurance is accepted, but not required.

Can I get vaccines at a CareVan?

Yes. Based on your needs, your provider may recommend vaccines during your visit. Vaccines may include seasonal flu, COVID-19, pneumonia, shingles, Tdap, Hepatitis B, HPV and more.

Can I get mental health or social service support?

Yes. The team can help connect you with behavioral health resources, social services, housing support, food resources, benefits assistance and ongoing care.

Do I need ID?

ID is recommended, but not required.
