How to make a referral

Please fax your referral and demographics sheet to our secure fax line: 860.870.0625.

A representative from our team will reach out to schedule an appointment with the patient.

Clinicians within Hartford HealthCare:

Refer patients through Epic by entering “Amb Referral to Neurology - Movement Disorders.”
